Scope and content: Winfield State Hospital and Training Hospital master patient index, on index cards and organized alphabetically by family name. Information contained on cards for individual patients may include patient name, guardian or correspondent and their relationship to patient, whether individual was a private patient, date of birth, discharge date(s) and date(s) of transfer, date admitted and age at admission, date of death if known, temporary visit and return dates, length of institutional stay, and other related information. Not all patients' cards provide as much information; earlier patients generally have less information. Index cards typically note a reel number, which corresponds with microfilmed patient files (see series UID 216259, Patient files from the years circa 1909-1984).
